Indications of urgent surgical treatment in disk herniation
1. Big picture
Most disk herniations are not surgical emergencies. Many patients with radicular pain improve with conservative treatment. The exam point is to know exactly when disk herniation becomes dangerous and requires urgent neurosurgical or spine surgical treatment.
The key idea:
Disk herniation needs urgent surgery when it threatens the spinal cord, cauda equina, conus medullaris, or causes severe/progressive neurological deficit.
Pain alone is usually not an urgent surgical indication. Pain plus progressive paresis, bladder/bowel dysfunction, saddle anesthesia, or myelopathy is urgent.
