Treatment of infertility
1. Big picture
Infertility treatment is not one treatment. It is a stepwise plan based on the cause:
Ovulation problem → induce ovulation Tubal blockage → surgery or in vitro fertilization Male factor → optimize semen / intrauterine insemination / IVF / intracytoplasmic sperm injection Uterine cavity lesion → hysteroscopic correction Unexplained infertility → expectant management, ovarian stimulation + IUI, or IVF depending on age and duration
The examiner wants to hear that infertility is treated as a couple problem, not only a female problem. Treatment should start only after basic evaluation of ovulation, semen, tubal patency, uterine cavity, ovarian reserve, age, and duration of infertility.
